CoE-IHS Webinar: Meadowlark – Building a Team-based Approach to Perinatal Care

The Meadowlark Initiative, a team-based model of care for pregnant and postpartum people and their supports, includes strong care coordination alongside integrated behavioral health. This model recognizes that pregnancy and postpartum are complex and dynamic periods that require a holistic approach to health care.

The Meadowlark Initiative prioritizes building relationships between health care providers and patients, so individuals receive care that is respectful, compassionate and culturally sensitive. It’s been shown to improve outcomes for pregnant and postpartum people, including reducing newborn drug exposure, improving maternal and neonatal outcomes, and reducing the need for foster care placement.

Join us on Wednesday, June 28, 2-3 p.m. ET to hear from the developers of the Meadowlark Initiative as they share more about their work in Montana, and demonstrate how this groundbreaking model is transforming care for pregnant and postpartum people.

After attending this webinar, you will be able to:

  • Understand and identify the core components of the Meadowlark Initiative in building a team-based approach to integrated perinatal care.
  • Recognize at least two health outcomes in integrated perinatal care associated with the Meadowlark Initiative.
  • Discuss changes in the integrated perinatal practice culture through utilization of the Meadowlark approach.

Don’t forget to register for the follow-up office hour session on July 27, 1-2 p.m. ET!
