High-Functioning Behavioral Health Team-based Care

This paper describes team-based care as a model of delivering mental health and substance use disorder treatment in community behavioral health clinics (CBHCs) to improve individual care, population health outcomes and the work experiences of all team members who provide care. The paper also presents five critical elements of the model that are necessary for high-functioning team-based care in the CBHC setting and actionable tips for implementation.
